Admission Policies
Admission Policies At Montessori School of Agoura
Admission to Montessori School of Agoura is open to children 2 years of age to 6 years of age whose parents or guardians satisfy the policies listed below. We suggest that all parents or guardians tour our facility. They must meet with the director or an administrator to obtain a registration packet and review our school program and policies. The registration packet which includes the following:
State Requirement
-Enrollment application
-Child’s Pre-Admission Health History form (to be completed by parent)
-Immunization record (copy of ALL vaccines)
-Consent for medical treatment
-Child’s Pre-Admission Physical Report (to be completed and signed by a physician)
-Parent’s Rights
-Personal Rights
-Receipt of School policies and Procedures
-ID Emergency form
School Requirements
-Health History form
-Tuition/ Admission Agreement
-Photo Release form
-Admission Agreement
